Voter Registration

WI Driver's License/WI ID Card:  Any person registering to vote must supply his or her Wisconsin Driver's License number or WI ID number.

If the elector does not have a WI issued Driver's License or WI ID Card, then the elector must provide the last four digits of their Social Security number.

If the elector doesn't have a current WI Driver's License, WI ID card or Social Security Number, this must be indicated on the Voter Registration Application.

Four Options to Register to Vote:

BY MAIL:  Download the complete Voter Registration Form (GAB 131) and mail it into the City Clerk's office at City of Ladysmith, P. O. Box 431, Ladysmith, WI  54848.   The application must be postmarked no later than the 20th day before the election and the voter must include "proof of residency" as outlined below.

IN PERSON: Register at City Hall up to 5:00 p.m. on the Friday before the election.  If registering during the late registration period which is 19 days before any election, the applicant must provide acceptable proof of residency as outlined below.

AT THE POLLING PLACE ON ELECTION DAY:  If you wish to register to vote at the polling place (Ladysmith Fire Hall), you must bring proof that you have lived at your present location for at least 10 days preceding the election.

Acceptable Proof of Residence:

The following constitute acceptable proof of residence if the document contains your current name and current residential address:

  1. Current and Valid WI Driver's License
  2. Current and valid WI ID Card
  3. Official ID card by a WI governmental body or unit
  4. ID card issued by employer with photograph
  5. Real estate tax bill or tax receipt for current year or year preceding date of election
  6. Residential lease which is effective for a period which includes the election day (NOT acceptable for first-time voters registering by mail)
  7. A photo University, college or tech school ID card
  8. A gas, electric or telephone statement (not earlier than 90 days before election day).
  9. Bank statement.
  10. Paycheck
  11. A check or other document issued by a unit of government.
